Fees

Individual Counseling:        $55.00-80.00/ 50 minute session
Marriage Therapy:               $65.00-85.00/ 50 minute session
Couples Counseling:            $65.00-85.00/ 50 minute session
Career Counseling:              $55.00-80.00/ 50 minute session

  • Mental health services are considered medical expenditures and funds stored in a Health Savings Account can be used to pay for services.
  • Clients wishing to use their insurance will be provided a Statement of Service so you may bill your insurance company direct. Please note the reimbursable amount is set by the insurance company.
  • Accepted forms of payment include: Cash, Check, Visa, Mastercard, Discover, and American Express.
  • ​Payment is required at the beginning of each session, unless special arrangements are made in advance.
  • Payment plans are available. For more information contact Angelie.
  • Cancellations must be made 24 hours in advance to avoid full charges.  ​​

Forms

New individuals and career clients please fill out the confidential Counseling Intake Form and Consent for Counseling Services, and bring these forms to the first session. New couples please fill out the confidential Couples Counseling Intake Form and Consent for Counseling Services, and bring to the first session. All new clients please print out a copy of the Professional Disclosure Statement and HIPPA Privacy Policy prior to the first session.
